Go For the Gold Health Risk Appraisals

How The Process Works

As part of our Go for the Gold Program, you have the opportunity to complete a health risk appraisal (HRA). The HRA is about 75 questions in length and should take only 10-15 minutes to complete. By completing the HRA during the period of March 1 through March 31, you’ll receive a completely customized report that provides you with important information and insight on how you can protect and maximize your personal health status. Remember, the HRA is available at no charge to Teledyne employees.

How Do I Take the HRA?

· Simply log on to www.wellstreamonline.com anytime from March 1 through March 31, 5:00 pm CST.

· Your Username will be “TBE + Your Clock Number” (e.g., TBE034500).

· Your Password is “wellness”. From there, you will simply need to follow the onscreen

instructions. You will be prompted to change your password before you take the HRA.

· At the completion of the HRA, you must click on “download your report”. You will then be able to view and print a personal report detailing your health risk status. Jennifer Geist, TBE Wellness Manager, will receive a report from WELCOA listing the participants’ names and dates when they completed the assessment.

Complete the HRA and the Screening and Receive $40.00

Because TBE believes that every employee should better understand their own personal health status, the company will give you $40.00 for completing the HRA and taking part in the on-site health screenings. This will earn you* a Bronze Medal in our Go for the Gold Program. Depending on how well you do on your health screening, you could earn a Silver Medal ($150.00) or a Gold Medal ($250.00). No one at Teledyne will see any personal information from this HRA. Again, the information that you gain from this offering is yours and yours alone. Teledyne will only receive a list of the participants’ names and the date they completed the assessment.
